Each referenced caremap has been evaluated by the site developers - a specialized geriatric services team at Sunnybrook Health Sciences Center. The repository was created with the assistance of an innovations grant from Sunnybrook and with the assistance of the Community Development in Aging & Health Project of the RGP of Toronto.
