The Regional Geriatric Program of Toronto
|
The RGP supports health care professionals in Toronto and surrounding regions in the provision of interdisciplinary, senior-friendly, and evidence-based care that optimizes the function and independence of seniors and their ability to age in place.
The RGP supports a network of 28 participating organizations whose services span the continuum of hospital and community based care. See our network map or to read more about our participating organizations click here RGP Network Organizations. To make a referral to a specialized geriatric service at one of the participating organizations see our Specialized Geriatric Services Inventoryto find the referral and contact information. Affiliated with the University of Toronto, the RGP builds capacity by making clinical tools and educational resources available to practitioners. |
